Things to Know
- Please arrive for your appointment at least 15 minutes early.
- Arrange for your primary care provider to send relevant medical records prior to your appointment. If necessary, bring x-rays, diagnostic images or other records with you to your appointment.
- Bring your current medication list (including all over-the-counter medications, inhalers, eye drops, etc.)
